Preparing for your Visit

Prior to Your Appointment:

After you book, new-patient paperwork is sent to you and completed through the patient portal. See Patient Forms for details. Due to privacy issues, please do not attempt to send forms to our office via e-mail.

Please check with your insurance company(s) to verify your coverage for routine examinations by calling the customer service phone number listed on your insurance card. You may also call this same number to verify “medical” benefits for medically necessary visits and procedures.

The Day of your Appointment

Bring your medical insurance card(s), current driver’s license or identification, HSA or credit card, and relevant medical record(s) with you. Please complete all of your paperwork in the patient portal prior to your upcoming appointment.
